My daughter did the same thing all the time. She looked so beat up from all the big scratches on her face. As she got older (she's 9 months now), I realized she was trying to rub her eyes. As your son gets older, his coordination will get better, and the scratching will stop because he'll actually be able to rub his eyes too!

Put little baby booties on his hands.
They actually make little mittens for this very reason, but they are designer and rather expensive... Just use booties and keep the nails trimmed.
I swaddled my kids until rather late... They were comfort and security freaks and didn't mind.
Does he like being swaddled? Wrapped up tight like when he came home from the hospital? I think it is the worst thing parents do to stop that too soon. Free hands make for scratches and well... trouble if you ask me. Not all the time mind you... just when they seem restless and fussy.
4 months is not too old... 5 or 6 yeah. That's when I stopped at about 5 months.

My baby girl does this tooo! :) Lol. Have you tried those baby socks on his hands instead of the mittens? Those mittens really are pretty useless!! We even have an ultrasound pic of her doing this same thing in the womb!!!
I do think that he will get better at clawing tho. My little one uses the back of her hands now (instead of her nails like she used to) and she's 5 months today.

Let him claw once or twice. Maybe only cut his nails every other time you feel inclined to. Once he figures out that "Ow, it hurts when I do that." He'll stop doing it and maybe use the back of his hands as another poster said. As long as he's not hitting his eyes or drawing blood, it shouldn't be much of a problem.

He may be clawing at his face because he itches. My daughter would always scratch on her thigh or behind he legs when she would itch.
It may be a good idea to change the ointment you are using too.
I would seek advice from a pediatrician to clarify what the problem may be.

yes when babies get frustrated they pull n e thing. my nephew used to pull out his hair. just put mittens on his hands

give him a small, plush, soft blanket or towel to hold on to when you realize he's getting sleepy.

what if you give him some kind of blanket or toy that he gets when he's tired, so he can 'pick' at it, or squeeze it. .. ooo, like one of those stress balls!! Try that!!
