In Internet Explorer, go to Tools menu > Options. Click the "Content" tab. Click the "AutoComplete" button. There are two buttons: one to clear forms (this is what you are looking for) and one to clear passwords (you might want to clear these as well!).

If you're using Internet Explorer, go to Tools, Internet Options, Content...
In the "Personal information" box, click on the AutoComplete button...
In the window that pops up, uncheck the boxes if you don't want to use AutoComplete at all. Click the buttons in the "Clear AutoComplete History" box to clear the currently stored data.
That should fix your problem.

If you are using Internet Explorer, go to Tools-->Internet Options. Under Browsing History, click Delete and click Delete Forms. That should do it for Internet Explorer. If you are not using IE, post back with Additional Details and I will further help you out.

Internet Explorer :
Select "Tools"
Select "Internet Options".
Open the "Content" tab.
Click the "AutoComplete" button
Remove check marks from the necessary check boxes.(will disable it)
Or click on clear forms...
Click OK.
To disable AutoComplete in Netscape Navigator:
Select "Edit" then "Preferences".
Open "Privacy & Security" category.
Select "Forms" sub-category.
Remove the check mark from the "Save form data from web pages when completing froms".
Click OK.
To clear AutoComplete in Mozilla Firefox:
Select "Tools" then "Internet Options".
Select the "Privacy" tab.
Open "Save form information" category.
Remove the check mark from the "Save information I enter in web page forms and the Search Bar".
Click OK.
