Well, it was General Motors (who own Chevrolet, Vauxhall/Opel, etc) until recently. They (and other American firms) are now being overtaken by a number of Japanese manufacturers, in particular, Toyota.
In America, which is obviously a massive car market, despite a perception that everyone wants a Humvee, there is a growing demend for smaller cars, with rising fuel costs and with more people banging on about climate change. Some motoring writers would add that American cars, in general, are badly made, especially when it comes to interior plastics, compared to Japanese and some European rivals.
It was the same during the fuel crisis in the 1970s. People traded their Chevrolet Impalas and the like in for Honda Civics and Toyota Corolla, and what did the American companies respond with? The AMC Pacer, which wasn't actually that small or economical, and simply couldn't compete with the better engineered foreign rivals. It then wouldn't be too long until the best selling car in the US was the Honda Accord.
It can be argued that an American weak point when it comes to car design is the small family car. Chevrolet did sell a small shopping car called the 'Metro', but that was really a badge-engineered Suzuki Swift, and I'm sure there's some more examples out there.

Depends on what measure you use.
I believe in terms of total profit Toyota is the best.
See results at http://www.toyota.co.jp/en/ir/financial_results/2006/index.html
However, Porsche makes more profit per vehicle sold.

Toyota. They get more back per £ spent making the car than anyone else. Probably one of the few manufacturers to be in growth, not decline.
