Frequently. As in 70% of the lovemaking sessions I enjoy with my wife.
Thing is it isn't difficult to achieve -- if a man has good control and pays attention to how quickly his lady goes those final few rungs up the 'O' ladder to what I call the cascade, i.e. that moment when she is going to 'O' no matter what.
Then it's a matter of shifting technique to where the man doesn't keep on pushing toward his own orgasm until she reaches the final countdown so to say.
It also helps to be a good enough lover that she gets to 'O' multiple times, so that even if she gets the first big ooh-la-la, there are plenty more opportunities for the simultaneous one.
Oh, and guys-- the longer you wait, the bigger the bang, and the biggest bang of all is when it happens simultaneously. So pay attention and be the good man in the bed, not the selfish one.

We've had some that way. They are definitely not always simultaneous though...Maybe one in every fifty times...

I did about two days ago and it was very very very very very good... With my husband it's always good. I'm one of the lucky ones...
