Lack of sleep, poor nutrition, learned dysfunction, family issues (these could be numerous), learning issues (dyslexia, needs glasses, hearing problems...), health issues, poor self esteem, bullies at school, probably more but...

This is a university class you're talking about? Just throw them out. It's not high school, and students are paying for the privilege to attend class, not the right.
I put a note in the syllabus that disruptive students will be shown the door.

Some kids do not get attention at home & act out in class for attention. They seek attention through negative behavior. As long as someone is acknowleging them, they are happy. Other kids get too much attention at home & are not taught limits, etc. so they feel that they can demand your attention whenever (the "look at me" syndrome. Their parents dote on them & baby them & they do not mature at the same rate as their peers.

They were never taught better at home. That is unless they are acting out (rebelling) about something else in their life.
