It's up to the publisher what type of information ends up on each page. Sometimes, it's the title of the book, sometimes, the chapter, sometimes the author. Most likely, accompanied by the page number. The publisher decides the layout.

I think it depends on the publisher, and probably what the author wants on his/her books as well.

Well, it's a little something called "I WANT CREDIT!!!" Everyone wants their name plastered on the TV and Papers for doing something good. So, if you own it, and you spent the time to write it, why not put your name on it? After all, it's yours! No one else made it!

not sure, perhaps it is easier to know whether people just take their pages after photocopying? but i think it's the author's preference..
