Usually the only test that you need is a blood test. But it really depends on the state that you live in. I would suggest that you contact the county court in the city where you live. The place that issues you the Wedding License should be able to tell you if you need to get a test done, and what test if any you need to get done.

depends on what state, some just require you to have MMR, some states don't require any type of blood test or vaccinations, call you local health department

In Pennsylvania, you need to be tested for STD's.
