I think maybe he posed for a painting, perhaps with his hand on his stomach, which may have been a popular portrait pose. You have to stand very still for a long time when being painted from life, so maybe it was to rest his arm that he slipped the hand inside the vest. The buttons would have supported the hand and arm...
The portrait became famous, and now everyone visualizes Napoleon with his hand in his vest!
Just be glad his hand(s) weren't down his pants.

He was simply posing for the painting. Does any other explanation really make sense ? Do you actually believe that he had a medical condition so awful that he couldn't take his hand off his body long enough to pose for a painting ? Even if he did, the painter would have painted his hand out of his uniform. The only good explanation is that he wanted it that way. Historians will tell you the same.

Napoleon died of an unidentified ailment, possibly stomach cancer; because traces of arsenic were later found in his remains, some have suggested he may have been fatally poisoned... The famous portrait of Napoleon with his hand stuck in his vest was painted by Jacques-Louis David.

According to one legend Napoleon was a very superstitious man, and kept a talisman inside his shirt,he would put his hand inside his shirt to make sure it was still there.
Many of the other theories sound good too.
The only thing I know for certain about Napoleon is that he had buttons put on the sleeves of his soilder's coats so they would not wipe their noses on them. (Ouch)

A couple of theories.
The most plausible one is that it was a 19th century affectation for men to pose that way (as they didn't have pockets).
The other, less plausible one is that he had a stomach ulcer/itch/breast cancer that he needed to touch to relieve the pain.

Yes indeed simply a common pose for having a painting done many many other examples of paintings in the time period of different men in this pose....You have to stand for hours
