What if you changed your name, if you tell your last boss will they most likely be cool about it & just use your new name as if that was the name they knew you as, or will they feel the need to tell the new boss that your old name was XXXX. ?
It's just a little embarassing (I mean changing first names, I switched my middle and first name).

There's nothing embarrassing about that. It's probably worth mentioning on the application, since otherwise it might cause confusion on any reference checks, but beyond that, don't worry about it.

They will most likely ask you on your application for any prior names anyway so you are going to have to tell them anyhow.
