Dying during child birth is rare with today's modern technology. Of course nothing in life is guaranteed so sometimes it does happen. Causes can be from toxemia - the high blood pressure can lead to seizures and/or stoke. Another cause is excessive bleeding including when the placenta prematurly separates (can be dangerous for mother and baby). If a c-section is needed there is a risk with this as it is classes as "major surgery" particularly when a general anisthetic is required in an emergancy situation.

In addition to uterine rupture, prolonged labor (especially if the baby is in a breech position) can cause stroke, heart arrhythmia and death to the mother. Hence the continued heart monitoring of both mother and baby in a hospital setting. Although dying during childbirth is rare, it can and does happen even with excellent medical care.
