Efferent nerves appear how nerve cells are typically drawn - short dendrites and a long axon. Almost all efferent nerve cell bodies are located in the central nervous system. Afferent nerves are structured opposite - they have long dendrites and a short axon. Almost all afferent cell bodies are located in the peripheral nervous system.

Nor do I ever recall hearing of a nerve multi-purposed in this way. If there is one, I'd like to know!

The direction of the nerve impulse is away from the cell body down the axon. If the impulse is going toward the CNS it is a sensory or afferent nerve, going away from the CNS to innervate either a muscle or gland is an efferent or motor nerve.
