I have to make a case management statement for a court case, but I don't see any information about what it is exactly and if there's a certain form I need to use?

The local court rules will specify what is to be included. generally it involves conferring with the opposing attorney & sets forth briefly what the case is about; what form of relief is claimed; whether a jury trial is demanded; whether there are any related cases in court; what issues & facts can be agreed upon & similar matters. If this is a Federal case, the requirements are in Rule 16 of the Federal Rules and the related Local Rules that each Federal district promulgates.
