This is a longer post, but you might find it interesting:

My theory is that wherever it came from (most likely a Dharma experiment), it is definitely the "monster" that can throw people a mile away, crush them, and rip them to shreds (like the pilot in the cockpit). It can take the form of anyone it gets close enough to copy, and can also take images from their memories to appear as people from their past (i.e.: Jack's father, Eko's brother, Kate's black horse). I don't think, with the exception of Hurley's Dave, that anybody is seeing things that aren't really there; I think that the monster toys with them in order to bait them -- like in the case of Eko.

Unfortunately for Eko, it took the form of his brother and didn't allow him to make peace with his death before it showed up in blob form and killed him. Just before he died, Eko told Lock that they were next. Why would their ally wish to make such threats? (If they were warnings, they'd have come out differently.)

I think it is because the monster has "absorbed" his essence, leaving his dead body behind with a piece of it in him. Whether this means that dead characters will rise from their graves, or we will see the monster take their form; I'm not yet sure. But I feel strongly that something along those lines is going to happen. And that they leave it incredibly ambiguous as to whether it is science(fiction) or supernatural forces that drive the island and the monster.

It has appeared in different episodes in previous. I think that it's a manifestation of the survivors' fears, past experiences and hopes all rolled into one. It seems to appear more when a character has something to confront from their past.
