Google your brand of garage door, they will show the different openers that go with it.
Order it online. If a company intalled or repairs your garage door there will be a sticker on the motor. Call the phone number and they can sell you a replacement. Or go to Home Depot with the info and buy one there.

You will need to do as said above.
After you get a new one it will need to work. You can try as is after you install a battery. If it work that good and bad. Here why.
If it work then your codes were never changed and anyone can buy an opener and get into your house when you gone. People actually do this. They drive around pushing the button until the door open on a house.
To change to code you may have your instructions for this. If not, normally when you open the remote opener you will see a little rectangle box with about 8 little switches on it. This has to be switched like the box on the opener itself. Which you would need to see what it is. But, if you have another remote opener it be easier to look inside it for the switch configuration. If I remember correct the switches have 3 positions (up, center, down for example) so pay attention.
HERE a better explain I found you and the reference;
You may set your transmitter to any code you desire but be sure that the code you set matches the code has been set on your radio receiver. The digital code is determined by the positions of the 10 small switches numbered 1 through 10 located in the receiver and transmitter. Any combination of ON (CLOSED) or OFF (OPEN) positions can be selected by using a small screwdriver or ball point pen. (NOTE: The switches are in the “On” position when the side next to the number is pressed in.) In setting your personal code we strongly urge that several coding schemes be avoided: ALL ON; ALL OFF; 2,4,6,8,10 ON; 1,3,5,7,9, ON. These positions are similar to our or other manufacturer’s test positions, or are frequently used. Once the code has been set, check the operation and replace the hatches.

Lost my portable garage door opener; what's the procedure to replace it?

You might be wondering how it works with your garage door. Inside the door opener there is usually a set of "pins" or switches set to a certain number or code. You simply match that to your new opener and you're all set. Just in case you needed to know that. That is why you can just go buy a universal replacement or the correct brand replacement. Good Luck :)

clearly the work of a poltergeist. Or some poor long lost soul that can't rest until it has fulfilled its life's destiny and opened your door. Come on dude it was clearly a draft. Also aren't ghosts supposed to be able to walk through walls so even if people were right about their existence (WHICH THEY ARE NOT) then why the hell would they open a door and not just walk straight through it?

You can buy them at Home Depot or another home store. Make sure you have the brand name and model number. You might have to go to a dealer for some brands - then I would look in the yellow pages for a dealer who carries your brand and they can replace it.
