normally it is somewhere between the middle and tip.....or the middle

It's pierced the same way as mens' tongues...about an inch from the tip.

You can do it in either spot. Some people choose to do it in the tip for purposes of giving better oral sex. They are gross though...just think about how much plaque those things must collect. *shudder*
