It is real ugly, I don't think it's real his head and shoulders don't match body...

grosse, yeah its probably real. because my brothers do body building and ive seen some of the videos they watch and the guys look like that. :S Although the picture is likely been airbrushed and slightly altered for perfection, but it is likely a true image of the guy.

Coming from a figure model who spends most of her days with bodybuilders I can promise you that picture is fake. I have had the priveledege of meeting Jay Cutler, Mr. Olympia and the best bodybuilder in the world, and he doesn't look like that.
The picture has very unrealistic proportions...don't be fooled

it certainly looks altered, but if the type that's religiously trained, lifted, and ate supplements like rice since birth, it's realistically possible
