Hi there...as the other person answered it's your hard drive icon which was accidentally renamed. You can rename it again to either the original "Macintosh HD" or anything of your choosing. Here's how to do this: select the area with a single click over the question mark and wait for it to highlight and next begin typing and then hit the return/enter key.

I think the standard default name was Macintosh HD but you said the cousin renamed it ?
You should still be able to click on it to get to all your programs, even with a different name.

It doesnt matter what its called, but the name it comes with is 'Macintosh HD' and many people rename it to say their name.. like 'Pats Mac' or something.
