Well. In personal experiences I say Grand Theft Auto has the best do anything of modern time. If you want mideval, fantasy-ish then get Morrowind or Oblivion. Personally haven't had the chance to try Oblivion but I know Morrowind had A LOT of stuff you can do. In fact... I never followed the storyline through.

When Pirates of the Burning Seas comes out in June 2007 that is going to be the best game ever made. Check it out its gonna be great.
