How do people address a Dictator in person, such as "Your Royal Highness?"
Are there different terminology for different positions, such as subject, servants, people of power, etc?
What are the protocols of etiquet?

In the US, we address him as "Mr. President," to his face, and behind his back, "That dumb-(anus) m/f..."

It depends. Some dictators are military men. Those would be referred to by their rank (General Pinochet). Others are just common citizens, and as such are called by whatever their particular government declares (President Mubarak). It varies, really.
