I admire your courage and dedication to improve your reading. This is one area in life that many people just get frustrated and try to deal with a lower reading level. Keep up the hard work and you will see the fruits of your efforts.
I recommend going to a bookstore like barnes and noble and asking a clerk to help you find books around a 5th or 6th grade level in a topic you like. Do you like murder mysteries? do you like stories about disibilities and overcoming hardships? What about biographies and historical fiction? You will have to remember that the stories were targeted for a lower reading level but I still find enjoyment reading kids books. Don't let large books scare you. Just take your time, use your dictionary and don't be afraid/ashamed to ask questions. If you don't understand what a paragraph/sentence is saying, ask someone you trust won't give you a hard time, such as a teacher or a coworker. Another thing that I have found handy are buying a printed book and the audio book (this can get pricey). This way, you can read the story and then listen to it to see if you understand. OR, you could listen to a chapter then read the chapter and maybe since you had an understanding already, you would be able to recognize the words better.
My biggest advice is to stay motivated and continue to work hard at it. Don't be afraid to ask for help and try different things. and finally, read stuff that you are interested in, that will help you want to continue to work at it.

Annie's baby
Secret life of bees

Go to the Library and ask if they have a reading tutor or know of some tutors?

The more you read the more you know.
the more you read the faster and better you will read.
