The country that is still going by the same name as when it was founded is: CHINA. It was unified under that name in about 225 B.C.E.
The oldest area of civilization is Mesopotamia, but of course it's now called Iraq. I think the first paragraph is what you meant.

I'm not sure, but I'd like to think it was ancient Greece or Egypt, though I don't know if the borders have stayed constant over the centuries. That's a very good question! I can't seem to find a straight answer on Google - the concensus seems to be San Marino, founded as a republic in AD 301.
