Yes there is a difference. You can be number 1 without having to be numbers 2 & 3. You can be number 2 without having to be 1 or 3. And you can be number 3 without having to be 1 or 2.
It's great if you could be all three things to that person.

1) The first one seems more authentic. He Loves you.
2) 2nd one means u are a girl friend. He may have a wife elsewhere or not yet married or no intentions to marry yet.
3) Means you Love him and vice versa is not clear yet. :-)
They may mean same and may be different. Just an opinion.

they are pretty different.
You are my lover: has sex attached to it, even if sex hasn't occurred yet. Sex is assumed.
You are my girlfriend: someone to be with, to go out with. A little bit of warmth there too.
You are my love: means you are the other side of his heart. That is the most important of the three I think.

In my Opinion:
You are my love means - person is in love with you'
You are my girlfriend - just means what it means - partner.
You are my lover - could just mean a f-friend....not necessarily in love or actually partners.
