If people are confused, it is often because they misheard something, or maybe you didn't say something in a clear way.

If you notice that puzzled expression on someone's face, do not react by panicking. The most important thing is to reassure them (with a smile, or somehow showing them that you got it that they didnt get it)... then repeat what you were trying to say, but make sure you say it a bit slower, and using simple vocabulary.

If you are trying to explain a process, explain it in numbered simple steps (number one, write your name... number two, write your address.... etc)

Another good way to help people feel less confused is to demonstrate what they should be doing. If they dont know what they are supposed to do, or why or how, it can be disconcerting. Say "like this..." and show them, and then make sure that they do the same.

Confusing isn't it?
First resolve your own confusion then it will be easier to resolve the confusion of the subject. Best try not to use words of more than two syllables when talking to the confused.

By improving your communication skills, ie., be certain that when you say or write something, it's in a way that can't be misunderstood. If it's something written, re-read it after writing and decide if you would really understand its meaning, had it been written by someone else. Also, when speaking, do so in good English, so that nobody can misconstrue what you've said. Don't talk 'in circles', get straight to the point.

Need a bit more info but..
Confused as in Alzheimer's type ailments then there are a few meds you can try like Ginko.
Confused as in people not understanding what you're saying then speak louder and clearer.

you make sure that you speak clearly and concisely. My old business studies teacher told me, provide your fact, back it up with evidence and move on! Don't waffle.
People who are confused and seek your advice will ultimatley still need to make their own decisions based on the above.
Provide them with an answer to a question and always make yourself clear. Ask them questions to ensure they have understood.
