It depends on what grade:
K-5: A game should be set up where they get a certain amount of warnings each day (usually 3), and everytime they get one, the teacher moves a clothespin on a "traffic light" looking thing. If it gets to red, the teacher should call home.
Middle School: If asked more than twice, should be sent out of the room or to the office.
High School: Given an F for participation and kicked out of the room (depending the severity of the incident).
College: Instantly kicked out / shot.
It's also important to reward the quiet kids, don't just assume that they SHOULD be quiet at all times -- make it known that it's appreciated; bring in candy.
