So far all maybe or possibly and last answer was an accident due to field bunching and not a safety car accident. Does anyone have a checkable answer?

Duncan S, if you check the 1995 Monaco Grand Prix web page it reports that in practice Taki Inoue was hit by a course car, safety car not mentioned. Tanks anyway for your input.

Seems to me there was some kind of incident with the Safety Car a few years back. Sorry, I don't remember what it was, or where it happened. The 1971 Indy 500 Pace Car certainly comes to mind though, plowing into a Grandstand causing numerous serious injuries.

As said previously, I think the only time was Monaco 1995 when Japanese driver Taki Inoue was hit from behind by the safety car during practise by a french rally driving nutter called Jean Ragnotti. Inoue was being towed back without his seatbelts on, the safety car hit his Footwork, it rolled over and he landed on his head, and wasn't fit enough to race!

Inoue also got hit by a marshalls car in Hungary 1995, because he was moronically trying to put the fire out on his car, despite being a moron. There's a video of that on YouTube.

I don't think if the safety car has been involved in any accidents, but course cars have. in 1995, I think, Japanese pay-driver Taki Inoue was run over by a course car coming to his help after he had retired due to a fire at his car. And in Brazil, in 2002, I think, Nick Heidfeld, in a Sauber, tried to squeeze between a course car(probably the medical one) and Enrique Bernoldi's Arrows. The car had the door open, and Heidfeld hit it, destroying the door and, I think, damaging his suspension.

A few years ago at (IIRC) Monza, the safety car slowed coming out of the Parabolica and the field bunched up; the drivers at the back didn't see it happenning and Montoya ended up spinning and hitting the barrier while avoiding another car.
