Size would be dependent on many factors, such as age, etc. But, from an article in PubMed:

"The authors measured the superior vena cava in the newborn by radiologic, anatomic and histologic methods and analysed the correlations between cross-sectional area and the biometric data. "

So, that is one process for how it may be determined.
