The penalty is 5 runs. It happened to Australia when playing against South Africa. It's so rare that it can be hardly seen.

These are called "Penalty Runs" and 5 of them are awarded to the batting side.
If the ball touches this helmet as it is lying on the ground, five penalty runs are awarded to the batting side. This scenario has happened quite a few times in all forms of cricket.
Five penalty runs are awarded to the batting team if a fielder fields the ball other than with his person (for example, using a cap or other item of clothing) or if the ball is touched by a fielder who has returned to the field without the umpire's permission.
This rule was introduced in the 19th century to prevent the unfair practice of a fielder using a hat (often a top hat) to take a catch.

Assuming that the batsman doesn't touch the ball with his bat or glove then any runs scored coming of anything else (helmet, leg, etc) then it is counted as byes and the runs go into the extras column. The batsman doesn't actually get the runs himself.
It's happens all the time.

Its 4 runs and the ball is immediately decared to be dead. which means that the batsman cant run or anything. It happened to australia earlier this year when they were in south africa. cant remember who the batsman was though.
