Tell her the honest truth.
After that you wait to see her response and you have to have enough self esteem to know you are ok no matter how she responds.

My advice would be to take a deep breath and just say something like, "I find you absolutely amazing. I would love to get to know you better. Maybe we could go out?" Or something along those lines. It's always best to be straightforward instead of beating around the bush. Girls are always trying to read more out of words than is necessary, so if you tell her straight up how you feel, there will be no question as to what you meant, etc. Good luck to you, hun. Be honest with her; but don't back her into a corner. Make sure you plan in going out for a drink--with her if the answer is good and with some friends in case she doesn't like you.

Have a glass of wine, or a shot of drink. It will keep your nerves from you skittering all over the place. I don't normally advise drinking (especially if your driving).

Aww...... good luck to you. Everyone will tell you to be yourself so I won't repeat it but just speak from your heart. Look down alot when you speak. That's cute to a girl cuz it shows her your kinda shy and that's attractive. Let us know how it turned out.
