it should be on the letter you recieved with the chip, it'll give you the default PIN and PUK. If you haven't got that then give T-Mobile a call....

Go online to T mobile website or just put tmobile puk code into google, sure you will find one easy enough...

It should've come with the phone. All phones are issued with their own PIN code & PUK code. Find the box with all your mobile instructions in it.

It depends how you mean 'blocked'.
If you blocked it because you lost it then you can get it at most mobile shops.
If you did an insurance job and the network blocked it - then you will not be able to release it. it's illegal.
