It's kind of like the phrase Kicking you when youre down. Basically that someone is rubbing the bad thing that happened in your face.

It basically means that the person already feels bad or embarrassed, then some smart a$$ comes along and says or does something to further insult them or make them feel bad, whether it be on purpose or not.

Its pretty self explanitory, if I kicked someone in the face and then spat on them that would be adding insult (spitting) to injury (being kicked in the face)

Example: Someone breaks up with you-injury, shows up with someone else the next day-insult.

To treat with gross insensitivity, insolence, or contemptuous rudeness.

Pretty much a person is way down on their luck and you come along and just be a total meanie to them not taking in to account how down they are.

it simply mean that you're already injured now you're getting insulted. In other words something bad has happened and now something else bad is happening.

Is it something like, adding salt to your wounds? It just hurts deeper.

iI think it's if you are hurting - and someone says the wrong thing maybe to try and make you feel better, but it comes out wrong and hurts you more.
