It's kind of like the phrase Kicking you when youre down. Basically that someone is rubbing the bad thing that happened in your face.

It basically means that the person already feels bad or embarrassed, then some smart a$$ comes along and says or does something to further insult them or make them feel bad, whether it be on purpose or not.

Its pretty self explanitory, if I kicked someone in the face and then spat on them that would be adding insult (spitting) to injury (being kicked in the face)

iI think it's if you are hurting - and someone says the wrong thing maybe to try and make you feel better, but it comes out wrong and hurts you more.
