These are two different beings. The grim reaper may be called the Ankou (a Celtic name). The _angel_ of death is Azrael.

All of those answers are quite good, with one exception. Charon (sorry Rico) is the boatman who transports lost souls over to the underworld. The practice of putting coins on the eyes of the dead is for them to have money to pay the boatman for their trip, long after Thanatos / Azrael has left.
