Office supply stores have leases that you can just fill in the blanks. Make sure it has what all you want in the lease though, If it does not have a addendum.

I think, you could get by with a simple agreement that contains provisions for:
Paying rent on time, paying for a portion of utilities, living room & kitchen rights, no smoking, no pets, no bf or gf overnight, no visitors after 10pm,stoarage space in garage?, car parking rules, use of washer / dryer.

The most important thing is that you feel comfortable about this person and that he/she is as clean as you when it comes to cleaning the house and washing dishes.
