Memory card trouble!?

Question

okay so, my memory card is in my LG V-CAST phone.
it lets me store videos on the card.
but, when I try to move pictures onto it, it says FAILED! or please insert card. when its already in. .
what do i do?

ive had the card for about 4 months.
it just started doing this.
