It needs washers & seals if it is an older style or a replacement valve if it is the newer single faucet style. Good luck....

Above correct probably just the rubber seals. Turn off the water to the house and open the handles to allow water to drain. On the handle the center a little cover that should come off with your fingernail, under is a Phillips screw, unscrew and remove the handle. Then you see the part you unscrew with a Crescent Wrench. You pull this out it may screw and then you see the rubber washer on the end held with a screw. Remove and take to a Hardware store or turn it over if it not worn on the other side and screw back on. Put back like you took apart.
NOW, one last thing. After you fix the washer if it old and it still leak it the insert that the washer seals is worn, They make a tool like an L shape and both ends have a tapered square that slide into the insert to remove, hardware store will know, remove the insert and take to the hardware store.
First link is Ace Hardware and show a simple Breakdown of the handle. Second link show the seat tool I speak of.

If you have a mixer valve for the shower, that's the problem. Remove the handle to the mixer valve and the round chrome cover. Behind where the handle was is the valve that's causing. There's a series of rubber O rings that are worn. You can buy the O rings and just replace them but I usually buy the whole insert with new O rings already on the valve. It's easy to do and only takes a screw driver and a large adjustable wrench or a big socket if you have one.
Oh ya, be sure the water is turned off when you're doing the job.
It you don't have one mixer valve in the middle and you have two valves, one for hot and one for cold, just take out the two valves and replace the rubber washers inside.

RE:
my tub spout leaks even when the water is turned off.?
Someone said that the valve needs to be replaced by going into it from the wall behind it. Is this the problem and any easy way to replace it?

snag is right the only thing is to remove the stem the valve u need a special tool to remove it its a hollow short tub that is like a wrench socket it is a plumbing stem revover like he said remove the srew on the end of the stem and take it to a hardware store to ger a new on u just put it back where it was i know that home depot has them because that is the store that i have used to repare bath hardware that is the problem because the only other seal is a washer that goes on the stem when u unscrew it from the unit and that ine inly makes the handle where the stem is at leak so i am preety sure that is the seal on the end of the stem because that is the one that blocks the water from going to the pipe to the faucent since its bad water goes throuth the damaged seal causeing water to go throught and leak to the fausent

if is just leaks for a few moments it is just pressure release in the pipes like an air bubble purging. If it is a continues leak the stem is leaking. is is hot or cold? If you replace the stem do not forget the seat inside yo need a removal tool. The tool cost less then 5.00 and the seat about 2 or 3 bucks. the stem. This is an easy fix a plumber will charge you at least 100.00 plus parts and you can do it for less then 20.00 You will also need a few other plumbing tools.
