At it's simplest level, it means that we don't yet have the technology or materials needed to prove something in real life that works on paper, viz., mathematically, by doing scientific equations. Historical examples have been the A bomb (just a theory in 1941) and putting a man on the moon (just a theory in 1961) both of which were eventually achieved.
Anything which worked in theory but could never work in practice would be a philosophical, speculative or imaginative exercise. World peace might be a good example (sadly).

It means essentially the same as: "Looks good on paper, but won't work in the field". In other words you can dream up perfectly good schemes in theory but when put in practice just won't work. The downfall of many an efficiency expert.

I think that when most people say that they mean that the creator of the theory in question has an unrealistic view of human nature. In effect, the speaker of that statement is saying, "If your conception of human nature were true, your idea would work. It seems obvious to me that your conception of human nature is not true (you expect people to be more self-disciplined, unselfish, consistent, patient, cooperative than is realistic to expect), so your idea will never work."

It means the person speaking acknowledges the "theoretical model" works, but thinks that the model oversimplified. However, that person can not or will not specify a more accurate model.

this is a false statement, because a theory is proven to work. "it may work in hypothesis, but not in practice" is better.
