What does it do to help you conceive?

Clomid basically tricks your body into thinking it's not making any estrogen. The body then responds by producing more estrogen, which leads to egg development, which leads to FSH and LH surge, which leads to ovulation.

Clomid helps you conceive by causing you to ovulate. Even if you were already ovulating, Clomid can help your body develop better quality eggs and lengthen your luteal phase, giving a fertilized egg a longer amount of time to implant. Clomid may also cause ovulation to happen sooner in your cycle than it normally would have, thus giving you more chances to get pregnant over the same period of time.

Clomid has a lot of side effects, and it's probably best if you take it at night just before you go to bed so that sleep through the side effects. You can expect to ovulate approximately 5-12 days after you take your last Clomid pill. I highly recommend charting your fertility signs at http://www.fertilityfriend.com and using ovulation predictor kits to help you time your lovemaking to have the best chances of getting pregnant.
