A man and a woman are driving along a country road on a cold winter night when the car breaks down. Before leaving to find help, the man tells the woman not to open the doors or windows and not to let anybody in. He leaves, and she does as he says. Sorry, guys, I hate to spoil it for you, but cos she kept all the doors & windows shut, the carbon monoxide fumes got in through the air vents & suffocated her. The "stranger" is the deadly carbon monoxide gas. No one said it had to be a human.

2 she was a guy with a full body disguise, the mask and stuff just looks like a dead woman

you dont say anything about the doors being locked!!! so my guess it that the stranger let him or herself in and killed the woman.
although the birth thing sounds great!!

there was someone hiding in the back seat all along.he or she sabotaged the car so it would break down.when buddy leaves he or she hacks up the chick and is stupid enough to stay.
