12 mins by Byron Moreno, he of Italy v Korea 2002 fame. He was officiating a game where the home team was losing 1-2 at the end of normal time. He let the game go on without indicating how much time he was adding.
The hosts eventually took a 3-2 lead after about 12 mins of injury, whereupon, Mr Moreno deemed it fit, to call a halt to proceedings.

He susbsequently retired after that game, at the ripe old age of 32!
