Besides all of the great answers from above here are a few things that might help you out.
You can download a map and guide of the Tokyo Metro Subway system from the website http://www.tokyometro.jp/e/index.html
CAUTION this is only the subway system. Tokyo is also served and supported very well by JR trains. The downside is JR does not provide a free map. You can purchase English versions of the JR train system from Kinokuniya Bookstores (SF, LA, Seattle, San Jose)
Most streets in Tokyo and Japan are not named like Elm Street and or Second Avenue. Only the large streets, highways and main arteries are named.
There are websites in Japanese and English that can be used to help find and navigate the best route and transfer points from station to station.
www.yahoo.co.jp has information but I am sorry to say it is only in Japanese.
