Chief of staff, that is, White House Chief of Staff, is chosen directly by the president, if the president decides to have one. It is basically the assistant of the president and doesn't have to exist. Currently it is held by Joshua Bolten.

All the rest of these positions are nominated by the president, and confirmed by the US Senate.

Director of the Office of Management and Budget: Rob Portman
Attorney General: Alberto R. Gonzales
Secretary of Homeland Security: Michael Chertoff
Secretary of State: Condoleezza Rice
Secretary of Transportation: Mary E. Peters
Secretary of Education: Margaret Spellings

Do you mean who decides who gets those jobs?

If that's it, they're for the most part appointees of the president.
Frankly, not sure about OMB. I think that's an appointee as well.
