Other good ones are Darth Vader in "Star Wars" (episode IV), and Jack Sparrow...sorry, CAPTAIN (!) Jack Sparrow in "Pirates of the Caribbean"

strangely, best movie entrances for me are not people, 1st - star wars episode 4 at begining (ships fly overhead), 2nd alien - popping out of gut. lol

The Deadly Viper Assassination Squad's arrival in "Kill Bill Vol.2" - starting with the long pullback from the wedding rehearsal, past Bill standing on The Bride's side and opening his jacket, back out the door of the chapel to the dooryard, and pausing as O-Ren, Budd, Vernita and Elle walk into frame on either side of the camera - which continues to pull up and back as they enter the chapel and the "kill-crazy rampage" ensues. This is one of the best single shots I've ever seen.

Not an actor entrance but what about the eats-leaves-asaurus (whatever) in the first Jurassic Park film. I remember being as bowled over as Sam Neil's character in the cinema because there hadn't been anything like that done at that point.
