Some can and some can not.

That would depend on whether the relevant vital organs are healthy and working properly.
All life must initially start with the ability to reproduce itself. Think hermaphrodites. The first so called human ancestors, contained both the female and the male genitalia. These earliest human ancestors mated and reproduced themselves. Individual preference and time determined which would eventually attain male or female status as we know it to be today. At conception all human life on planet earth develops along this same female dominant body structure line. Differentiation into male comes much later in fetal development. Human males still now retain these early features (breast ) although as non functional structures. Some modern humans still carry the same physical body structure as the earliest of our human ancestors.

Such people (more commonly called intersexed these days, tho some do still call them hermaphrodites) are usually sterile, tho there may be exceptions, and they're usually considered to be whichever gender they present as in ordinary life.

Usually, the parents decide what they want the kid to be after surgery, and most of the time they will end up being sterile.

The person is refered to as a hermophadite... and yes they can ejaculate and depending they should be able to have kids.

If he has a male organ "down there", he is a male. If she has an unusually large, protruding female organ "down there" that looks like a male organ, she is still a female.
