My husband and I filled one out when we met with a priest at a Catholic church. It was a requirement for the priest to marry us, so I'm sure that all kinds of Catholic churches have them. If you're not Catholic, I'm sure if you made some calls and explained your situation they would be happy to let you look at the test (even though you are not getting married at that particular church). I would also like to note that the test is more about considering what your married future has in store for you (finances, family planning) than religious stuff. It's a great way to generate dialogue!
