To a certain degree, I think men are buying into the better body image trap. I see this more and more with my friends.
I'm not talking about a basic desire to look good and feel good and be healthy, I'm talking about a need to subscribe to some imaginary perception of how someone is supposed to look, what someone is supposed to wear, etc.
Its one thing to work out for fun, fitness and improved self-confidence and health. But more and more I see my guy friends hitting the gym because its expected of them.
On the extreme side of this, I have two male friends who are very diet conscious, neither through a desire for healthy living but both for appearance-related reasons. One of these guys is on the Atkins diet, constantly, and he's not a big guy to begin with (5'10", 165-170 lbs) with an active, physical job. He is also a bit of a fashion plate - he'll buy designer clothing, cologne and accessories because this actor or that singer endorse the product, or some actress or model has stated she loves men who just.... fill in the blanks. The other hits the gym on a daily basis, preceded and followed by a slightly frightening vitamin and protein drink/pill regimen, all on the basis that no one will look at him twice if he's not in amazing shape (currently 5'11" and 175-180)
For the second part - no, not all women want men with a hard body. There's a point at which too much muscle becomes unattractive. And personally, I don't trust men who are really hot. Oh, they might be nice to look at, but I'm not much into guys that are all look and no substance.
Appearance doesn't matter nearly as much as hygene, and total fitness isn't more important than general health and a positive outlook. And its just not fun hanging out with people who are fixated on what everyone else things of them.

Not only TV, but the movies, what you read, billboards, everything plays into that thought that everyone everywhere is supposed to be in perfect shape, have perfect hair, perfect faces, and live a perfect life.. If that were the case, the world would be rather dull and boring. I like the fact everyone looks, acts, and lives differently.

Absolutely. Men feel the same way women feel about the way they look. Being in shape for men who do not precipitate in athletics keep their shape to be attractive for females. To feel accepted by society. Some of the younger generation act like girls. They shave their legs and arms. They get their backs waxed along with there eyebrows. Its funny to me. Lots of women like their men to look like men.

Yes, as are women. Not only TV, but the entire advertising and entertainment media are responsible.
