If his foreskin isn't so tight that it's causing him pain upon erection, he can try several things before considering any kind of surgery.

Stretching exercises work in the majority of cases. Take two fingers and stretch the opening of the foreskin as far as it'll go without pain (like increasing its radius) by hooking a finger on either side of the opening. It's a bit difficult to describe and make take a few tries to get right. But do this for a few minutes each day for several weeks to a month. If there's no improvement after a month or so, go to the doctor and ask about prescription creams that're supposed to help along with this exercise. It's important that he stretch it as much as possible without pain, because if it causes pain it could actually make things worse.

There are many surgical options, with a full circumcision being the most invasive. There is something called a "dorsal cut" where the end of the foreskin is split and allowed to heal. There is an operation to simply enlarge the opening of the foreskin. And there is also a partial circumcision where only the end of the foreskin is removed, so there may be enough left to cover, either completely or partially, the glans depending on the original length of the foreskin. Circumcision should only be considered when all other options fail.

In any case, try the stretching exercises first because if his foreskin isn't so tight that it's causing him discomfort/pain with an erection, it should work. Look at links for more details.

Is it possible for him to move his foreskin back at all? If not, the other procedures mentioned here will in the long run cause more pain than they are worth. You don't need to be afraid of circumcision. You will not regret it. It will prevent continuous infection under the foreskin. It will not diminish your sex life. It will enhance it. Adult circumcision is performed many, many times. Under local painkiller it is painless. Healing is in 2 weeks or less. It is done as an outpatient. There is some discomfort also controlled by oral painkillers. Go for it. Check out what they do on the web site. www.circumcisioncenter.com They are more expensive than others. Check around your local area for urologists or, even if you are not Jewish, for a mohel who does hundreds and may have a lower charge. Check a local rabbi or two about a mohel they use.

Fforeskin is designed to cover and protect the glans/head of the penis and even though it can be retracted for cleaning and when putting on a condom or durign sexual activity.
If he cannot retract his foreskin to fully expose the head of his penis, then he needs to do some stretching to loosen it up enough to be easily retracted. Once you are well, into puberty, the foreskin should be retracted for cleaning on a daily basis; more often if you are sexually active or participate in sports or other activities that get you all hot and sweaty.

There are several web sites (see below) that offer instruction on stretching as well as suggesting topical, steroidal ointments that can help if stretching by itself isn't quite working. As for cleaning, whether your foreskin retracts fully or not, always retract it as far as it will go and wash everything thoroughly with warm water and soap then rinse off all the soap residue before drying the genital area thoroughly.

If you work at the stretching daily, it will produce results in a few months. If it seems to be taking too long ask your pharmacist or doctor for one of the ointments that will help.

If it isn't too tight, you can try enlarging it a little bit.
Try putting something inside (wad for example) to keep costantly between the foreskin and the glans. Realize it needs many times, while surgery is easy and fast.

let him trim his fingernails first very short. then using plenty of Vaseline, slip his finger gently under the foreskin - between the foreskin & the glans. using the vaseline , let him push it in & wiggle it in gently & slowly till it can't go further, then he must twirl the finger all around the head of the prick - slowly &gently. Do this daily - GRADUALLY that skin will stretch & relax.

but use lots of Vaseline, cut nails very short, & go slow & gentle - & be patient. He'll get there. Best to do this while he's erect.
