...that's what makes them an epic story. All epics, classics must have an epic hero...he/she may have fault or nuiances or may not be liked by all...yet there is an underlying quality to their character that will do the right thing at the right time that overrides anything they may may have done before, or any character faults...
generally its self-sacrifice, or courage in the most testing of circumstances...the one thing that gets them over the line in everyones mind as a hero...
Regarding fiction stories, especially newer ones & through a different approach/mainly advertising, they try to make that epic hero, an anti-hero...but the fact remains they have that undelying quality in the content of their character that makes them an epic hero...
...you know it's an epic when u just can't wait to turn the page or for the next scene to see how that epic hero will show us all what is required...and we all wish it wish us...
...good question.

Of course! The epic literature must have great heroes by definition, and is often of great philosophical and religious importance to the native culture.
Some of the great epic literature:
The Mahabharata with the dynasties of the Pandavas and Kaurabas; Rama in the Ramayana; Achilles in the Iliad; and Sigfried in the Nibelungenlied

Traditionally, I'd say yes. However, you also have to consider the concept of the anti-hero; a character who may be a villian, an unwilling hero, or even cowardly. This is quite prevalent in modern culture, and not so much in older stories.
