I've worked in several restaurants, and have never seen this happen. It would be a violation of multiple health codes. Not saying that it hasn't happened somewhere in the world, but I've never seen it personally.

I doubt it. It would be a health code violation. If you know of one doing this, report them. A customer could contaminate a piece of bread with disease microorgansims or chemical toxins of some kind and wind up poisoning someone's salad.
A general rule of restaurants is that nothing that has touched the customer's food is allowed back into the main food supply. Most buffets will require you to get a new plate each time you go back because the servingware from the buffet might transfer germs back from the used plate to the food supply.
America has always had the cleanest food supply in the world and takes food sanitation very seriously.

Typically, leftover bread and rolls that have never left the kitchen will be used for next day's croutons, dressings and crumbs. No need to use bread from tables for heavens sake.

In the restaurants that I have worked in, we have not ever used food off of a table and "recycled" it. They have used bread not served, and made it in to bread crumbs, and croutons.
But never off of a table.
I have seen bad stuff though, so I am sure that it would happen somewhere.
But I don't think common is the norm. I think that is the exception

Not supposed to for health reasons. but some might risk it. The more common practice is to use the bread leftover at the end of the day and leave it out to dry.

i don't know ! but i hope it is not true, because i love those croutons in my soup or salads. they may use leftover bread, but definitely not those that were served to others before i hope ! that would be so friggin gross !

yes.
They also cook a lot of soup and cool it and just reheat it for "soup of the day"
Also if the fruits have gone off, they just slice the fungus and make it "fresh fruit platter" and "freshly squeezed juice"
Also all leftover pieces of meat, they just cut in little2 pieces for "pizza toppings"
And many other things. Wait till you hear what they do with beverages, he he he...
