You need more than a consent form. There are very specific rules for underage workers. Employers must conform to juvenile work guidelines, one of which is an Underage work permit, not just a consent form. Check at your local court house for Regulations and permits.

I also live in IL and when I was fifteen and getting ready to work my first job at McDonald's, I actually got the "work permit" through my school. I believe I needed a letter from the job willing to hire me at that age and brought it to school and got the paper work I needed.

When I was 15 the high school guidance couselors office provided the form for my parents to sign.

Call your local labor board or unemployment office, they should be able to help you. Or you could call your child's school office, maybe they could help you, but I doubt this because most of the time they can't help the kids they are in charge of giving a good education.

I just got working papers today (first one)

I got them from my school. My employer said usually the counselor would have it, but the lady at the front desk had working papers for me.

In Michigan, the student gets it from his or her school guidance counselor.

at his or her high school...that's where i got mine when i was 15 and that was only 6 years ago, so i'm sure they're still there
