paper work to fill out, does that mean i dont get any money????????

Disability either comes from Social Security - SSDI which you file through Social Security. Normally they deny the first submission and you will need to appeal.
Disability also comes from an employer through Labor and Industries in your state or a private policy (rare scenario).
The doctor's responsibility is to determine your condition. The paperwork comes from the agencies paying.

I have to get the form from my employer and take it to the doctor to have him fill it out. Then I make sure it gets back to my employer. The doctor's office usually charges 10 or 15 dollars for this. I had to do this several times this year as I was laid up all summer.
