Here is the MLA format:

If it's a book by one author, you put the author's last name first, then his or her first name and a period. Leave a space. Then put the title of the book. Capitalize where appropriate, and underline the title of the book. Leave a space. Put the name of the city where it was published, followed by a colon and two blank spaces. Then put the name of the publisher, followed by a comma, and the year it was published. If the copy you are reading has a lot of years, use the most recent one.

Start all the way out at your margin for the first line of your citation. If you need additional lines to complete the citation (you will probably need one additional line) then indent that line five spaces. Here's an model for you (I can't underline and it is not letting me indent the second line five spaces....) Sorry if it doesn't come out right.

Lee, Harper. Title of the Book, Underlined. City: Publisher, Year.
